Antibiotics vs an antimicrobial dressing for pin-track infection

چکیده
Background: Pin-site infection is a common complication in the treatment of fragility fractures of the distal radius in the older population. There is a need for evidence-based practice guidelines for pin-site care as few studies have compared the associated infection rates of different treatment methods. Objectives: A clinical study was conducted over a 24-month period to record the clinical outcome in patients treated with Acticoat 7, a nanocrystalline silver-release dressing, and OpSite Post-Op, a barrier dressing, in the management of pin-site infection. A secondary objective was to compare these outcomes with those of patients treated with oral antimicrobials. Methods:Twenty-one outpatients with a K-wire fixation for osteoporotic distal radius fracture with established pin-track infection were studied. The first 10 patients were treated with a one-week course of an appropriate antibiotic determined by culture and the rest were treated with Acticoat 7 and OpSite Post-Op dressings. Results:All patients in both groups healed completely, although the healing time was variable. Conclusions:The results from this study confirm that a nanocrystalline silver-release dressing may be effective as an alternative to oral antimicrobials in treating postoperative pin-site infection in otherwise uncomplicated patients. Further randomised clinical study is, however, required. Conflict of interest:None Abstract

Efficacy of 1 % silver sulphadiazine dressings in preventing infection of external fixation pin-tracks: a randomized study
Pin-track infection (PTI) is a common complication of external fixation. Antimicrobial dressings of the pin-site interface should reduce the severity and incidence of PTI. This study is aimed at determining the efficacy of 1 % silver sulphadiazine dressings in preventing PTI in external fixation. We compared the incidence of PTI between group A (dry sterile gauze dressing) and group B (1 % silv...
متن کاملUndisturbed theatre dressing during the first postoperative week. A benefit in the treatment by external fixation: a cohort study
In the literature, there are several different suggestions as to when the first postoperative pin-site care should be carried out to best prevent pin-site infections during the treatment by external fixation. In a cohort study, we compared the use of antibiotics and complications in patients where the theatre dressing was changed during the first postoperative week with patients where the theat...
متن کاملAntimicrobial gauze as a dressing reduces pin site infection: a randomized controlled trial.
BACKGROUND Pin site infection is a common problem in external fixation. Plain gauze wetted with normal saline is commonly used for a pin site dressing owing to the simplicity and low cost. Evidence to support adding an antimicrobial agent in the dressing material is lacking. QUESTIONS/PURPOSES We compared the rate of pin tract infection using plain gauze and gauze impregnated with polyhexamet...
متن کاملSemi-IPN Films and Electrospun Nanofibers Based On Chitosan/PVA as an Antibacterial Wound Dressing
The antimicrobial activity of a wound dressing is a key factor for preventing and treating wound infection. The current study evaluated the physiochemical properties and antimicrobial activities of semi-IPNs (interpenetrating polymer networks) based on chitosan/polyvinyl alcohol (PVA) films and nanofibers as candidates for wound dressings and investigated the effects of morphologies (nanofibrou...
متن کاملPrevention of pin site infection in external fixation: a review of the literature
Pin site infections are a common complication of external fixation that places a significant burden on the patient and healthcare system. Such infections increase the number of clinic visits required during a patient's course of treatment, can result in the need for additional treatment including antibiotics and surgery, and most importantly can compromise patient outcomes should osteomyelitis ...
